The converter reads the text on each page, along with the position and size of every word. It groups words into lines and splits two-column pages so they read in order.
Then it sorts the lines. Text much larger than the body becomes a heading, bullet lines become a list, and lines that form a grid become a table. Characters that Markdown uses for formatting, like * and _, are escaped so your text shows exactly as written.
The converter rebuilds the structure of your PDF from the size and position of its text.

Images, links, colors, bold and italic are not carried over. Numbered lists stay as plain text, so their numbers don't change.

A few quick checks make the output cleaner.
Use a PDF with selectable text. If you can't highlight words in your PDF viewer, run OCR PDF first.
Convert only the pages you need. A page range skips covers, blank pages and appendices.
